Who Can Play: Players whose birthdates are between August 1, 1990 through July 31, 2004, are eligible to sign-up.
All Registrations will take place at the TYSA office located at 1769 Carnegie
(Carnegie dead-ends into Fulkerth, between Tully and Golden State Blvd.)

Wednesday, April 9th 5:30-8:30 p.m.
Saturday, April 19th 1:00-5:00 p.m.
Wednesday, May 7th 5:30-8:30 p.m.
Saturday, May 17th 1:00-5:00 p.m.
Wednesday, June 4th 5:30-8:30 p.m.
Saturday, June 14th 1:00-5:00 p.m.
Wednesday, June 25th 5:30-8:30p.m.
Wednesday, July 2nd 5:30-8:30 pm

Fee: Under 6 Under 7 – Under 18

Payments plans are available. You must pay $__50.00___ per player at the time of registration. Payment must be paid in full by June 25, 2008.
Requirements: Every player needs to complete a Player registration form. The registration form must have a parent or legal guardian signature on the code of conduct and the consent for medical treatment signature lines.
· New to TYSA or did not play in 2007?
1. Bring your child’s birth certificate
2. U12-U14 players will need a current 1”x 1” picture
· Returning to TYSA from 2007 season?
1. U12-U14 players will need a 1”x 1” picture
THIS YEAR FOR ALL U15-U18 PLAYERS
• Whether you are a returning player or not you will need:
1. Will need to bring birth certificate
2. Will need to bring drivers license or student ID
(must have current photo ID)
3. Will need to bring a current picture to registration 1”x 1”

Team Sponsorship Opportunities: (See our website at TurlockYouthSoccer.org)
The fee to sponsor a team is $250.00. A web Sponsorship is $50.00. Sponsorship forms are available at the TYSA office and at registration. When forming teams, TYSA honors only the request of the head coach, the assistant coach, and the sponsor of the team for their children
